Social Prescribers are here to help you to connect with community activities, groups and services to support you with your health and wellbeing. They can discuss your lifestlye and preferences with you, and can connect you to local community activities, groups and services, as well as link you in with services who can provide benefits or energy saving advice.
Social Prescribers will work with you over a fixed number of appointments to help you to achieve your health and wellbeing goals.
Social Prescribers are here to help you with non-health related problems, including, but not limited to:

Social Prescribers might also refer you into local services that can help you to live well with your permission. They will work with you on a shared management plan to help you to improve your wellbeing.
