Living Well Community Wellness and Empowerment Service

The Live Well Empowerment Service gives adults living in Havering wellbeing support in the community. The service is commissioned by Havering Council and delivered by Tapestry Care UK through community hubs in Rainham, Harold Hill and Hornchurch.

 

This service will start on 1 February 2026.

What does 'Live Well' mean?

'Live Well' is a term used in the NHS and Council. It describes how we want residents aged 18-49 to live their best lives in the community, without needing long-term support from the NHS and social care where possible.

 

This services is not run by Live Well Havering, but by Tapestry Care.

What is the Living Well Community Wellness and Empowerment Service?

Living Well Community Wellness & Empowerment Service is designed to support people earlier, ensuring life’s hurdles do not become overwhelming.

organisations, the Living Well Community Wellness & Empowerment Service provides a single, point of access for support.

Everyone’s situation is different. The service starts with a friendly conversation to understand what matters to you and then helps connect you to the most appropriate support.

Who do we support?

We support:

  • Havering residents
  • Aged 18 and over who are not already getting support from Havering Adult Social Care

 

Who may want support with things like:

  • loneliness, isolation and connecting into the community
  • stress, anxiety and / or low-mood
  • caring responsibilities
  • neurodiversity
  • money worries
  • long-term health conditions

What support is available?

Support may include:

  • wellbeing and confidence-building activities
  • social groups and community connections
  • mental health and peer support
  • food, nutrition and practical advice
  • digital help (phones, online services, forms)
  • support and advice for carers

Support may be one-to-one, group-based, or delivered through trusted local organisations.

This support will be available across Havering through Tapestry’s Community HÜBs in Hornchurch, Rainham and Harold Hill, with outreach into Romford and surrounding areas to offer additional locations.

Once you have made a referral, it will be triaged promptly and a Live Well Practitioner will contact the resident to do a light-touch assessment to see what support they may like.
