We give care to people in their homes who are experiencing a physical health crisis that does not warrant a hospital admission.

  • Service operates 7 days a week, 365 days a year. 8am – 10pm.
  • Service is available to adults over the age of 18 in the boroughs of Barking and Dagenham, Havering and Redbridge
  • Service consists of from doctors, nurses (some are prescribers), pharmacist, occupational therapists, physiotherapists, social workers, health care assistants & administrators.
  • We work closely with many other specialist teams in the community.

 

Who can we support?

Any health, functional or social crisis where CTT intervention on the same day will prevent an unnecessary acute hospital admission, for example

  • A fall
  • Experiencing confusion/delirium
  • Reduced mobility
  • Infections (e.g. chest, urine, lower leg cellulitis)
  • urgent equipment provision

This is not an exhaustive list – they are aspects of common clinical conditions, or needs, that may lead to a patient requiring a two-hour response in a crisis.

  • A medical 999 emergency, CVA, Cardiac Arrest, MI
  • Rashes
  • Ophthalmology emergencies
  • Chronic conditions management
  • Routine blood tests
  • Vaccines
  • Suspected fractures
